To render
professional social work services
within aSubstance Abuse Programme
, with specific responsibility for thedevelopment, coordination, and implementation of Aftercare Services
. The post includesestablishing referral pathways and partnerships
within-patient substance abuse treatment centres
across the Western Cape, as well as supporting beneficiaries transitioning from both in-patient and out-patient programmes.Key Performance Areas
Render
professional social work services
in accordance withsocial work norms and standards
Implement and manage
Aftercare Services
for service users exiting substance abuse treatment Conductintake assessments
, case management, aftercare planning, and follow-ups Establish and maintainformal referral networks
with accredited in-patient substance abuse treatment centres Facilitate referrals from in-patient treatment into structured aftercare programmes Provideindividual, group, and family interventions
Ensure properdocumentation, case files, reports, and statistics
as required by DSD Participate inmonitoring, evaluation, and programme reporting
Ensure adherence toethical standards
, policies, and relevant legislationMinimum Requirements
Bachelor of Social Work (BSW)
degree from an accredited institutionRegistration with the South African Council for Social Service Professions (SACSSP)
- compulsory Knowledge and understanding of:Prevention of and Treatment for Substance Abuse Act (Act 70 of 2008)
Children's Act (Act 38 of 2005)
DSD norms and standards for substance abuse treatment services Aftercare and reintegration modelsValid driver's licence
