TB HIV Care (THC), in existence since 1929, is an internationally recognised non-profit organisation. We provide health care services throughout South Africa to both the general population and communities at increased risk for HIV and TB, including adolescent girls and young women, sex workers and people who use drugs. Purpose of the position: To provide professional and holistic social work services by assessing the needs of patients, families, carers and children with TB and/or HIV. The role involves implementing strategies and processes to meet these needs.

Minimum Requirements

  • Grade 12 or equivalent
  • Certificate in Social Auxiliary Work from a recognized college or institution
  • Minimum 2 years relevant experience
  • Registration with the South African Council for Social Service Professions (SACSSP)
  • Proficiency in English and isiXhosa languages
  • Clear criminal record
  • Computer literate in MS office advantageous

Duties and Responsibilities

  • Assists in the coordination of comprehensive psychological assessments of patients and their families at Anti Retroviral Clinics/ Mobile
  • Educates patients and their families
  • Informs patients and their families on eligibility criteria for social support grants
  • Networks with all relevant stakeholders and implements an effective referral system to ensure effective service delivery
  • Lobbies and advocates for patientxe2x80x99s needs
  • Provide appropriate interventions and/or feedback at the case management meeting on the needs arising
  • Follows up on referrals from hospital staff and/or teams
  • Submits monthly reports to the supervisor
  • Records and maintains accurate and detailed patient information and interventions and keeps statistics in order to submit as required
