Job Description


Main purpose of the job:

  • To provide oversight and technical support for the retention of HIV/TB patients and innovation services in the allocated sub-districts by working with the sub-districts and facility managers, Wits RHI community teams, community partners, community-based organizations, and linkage hub to ensure that clients tested positive for HIV/TB are retained in care in line with the 95 95 95 strategy

Key performance areas:
  • Review and align the Retention implementation/operational plan for the sub-district/selected facilities in line with the district and national strategies with the support of the Retention/QI Programme Manager
  • Review and align the tracking and tracing activities in selected sites for treatment initiation or follow-up visits for HIV/TB patients, including source documents and database updates by working with the different stakeholders (Treatment navigators, community teams and partners, PHC teams)
  • Provide technical support in collaboration with SI to ensure managers are monitoring the capturing of lost to-care patients from relevant paper-based systems by the relevant stakeholders, this includes the liaison with the relevant program stakeholders and teams to ensure that daily, reports are generated for early and late missed appointments as per the reporting timelines
  • Provide strategic guidance to managers to ensure that clients waiting for ART (for various reasons) and TB treatment and uLTFU lists from Tier.net are followed up back into care and treatment by working with the different relevant stakeholders
  • Oversee that selected sites have smooth referral and transfer in and out of clients within facilities to ensure retention in care and treatment
  • Liaise with Wits RHI community staff, selected facilities, and DOH staff about TB and HIV client management in terms of linkage and retention in care and treatment, etc.
  • Establish and maintain effective working relationships with key external partners to coordinate the program, NGOs, CBOs/ NPOs, FBOs, WBPHCOTs DOH, Treatment Navigators, etc.
  • Establish and maintain effective working relationships with key internal role players (Decanting teams, community teams, Pharmaceuticals team, contracted CBOs, Training/ capacity building unit, PHC teams) to coordinate linkage and retention activities in the selected facilities
  • Liaise with Wits RHI internal departments/units in the coordination of services for the outputs/ key deliverables for the program (AFYS, Decanting teams, PHC teams, Linkage hub, Wits RHI community teams, contracted CBO, WBPHCOT staff, treatment Navigators, etc.
  • Participate and contribute to Phuthuma activities within the selected Clusters
  • Provide feedback to internal on the retention progress of the supported facilities
  • Initiate and support the development of innovative strategies in line with the 95-95-95 strategy using available resources and technical knowledge
  • Guide innovative activities implemented by managers in allocated clusters to ensure that patients are retained in care
  • Provide strategic leadership and direction to all innovation discussions and activities while leading the institution of an innovation hub or team with the objectives of improving patient care and retention
  • Formalise and document innovations into CQIPs for reporting and presentation internally and externally (DoH)
  • Identify training needs and provide technical support for the development of linkage to the care curriculum
  • Support the capacitation of sub-district stakeholders (both internal and external) on the linkage and patient retention strategy in the subdistrict
  • Monitor stakeholders to ensure that patients are linked and retained in care and treatment services for all newly diagnosed and loss to follow-up HIV and TB patients
  • Oversee the mentoring of the various stakeholders on the linkage and retention activities in sub- districts
  • Provide technical support to the sub-district regarding linkage and retention strategies i.e. decanting unit, hospital, PHC
  • Ensure that teams and different stakeholders report their activities as per the tools for the program
  • Ensure and monitor the use of data collection and reporting tools as set out for the program
  • Ensure that data bases, dashboards, and any kind of reporting format are updated and customized for linkage and retention activities
Required minimum education and training:
  • A Nursing Degree or equivalent
  • A Masterxe2x80x99s Degree in Public Health as an added advantage
  • A valid driverxe2x80x99s license
  • Proficiency in MS Office
Required minimum work experience:
  • At least 2 years experience in working at a facility and community with knowledge and experience in primary health care

Demands of the job:
  • Working overtime or over weekends may be required from time to time
  • Able to work in a highly pressurized and challenging environment
  • May be required to work at sites that are under confidentiality, tact with discretion to be always maintained
  • Local travel may be required

  • Maintain effective working and communication relationships with the different stakeholders (Facilities, community partners, and organizations)
