Are you a compassionate and skilled Professional Nurse seeking an exciting opportunity to provide hospital-level care to acute patients in the comfort of their homes? Our client, a leader in the private healthcare industry, invites you to join their dynamic team.

This is an opportunity to do part-time home visits for patients choosing home-based care over traditional hospital settings. You will provide a service to patients living in one of the following areas in the Western Cape (within the area you live):

  • Cape Town
  • Stellenbosch
  • Hermanus

Requirements
  • A valid driving licence and a reliable vehicle.
  • Degree / Diploma leading to qualification as a Professional or General Nurse.
  • Post Graduate Certificate / Diploma in Emergency Nursing and/or Community Health Nursing - will be advantageous.
  • Basic Life Support certificate.
  • Valid SANC licence for 2024.
  • 3 years experience in a hospital setting (post-licensing at SANC).
  • 1 year in a home care nursing environment, experience in emergency care and experience in the care of medical patients will be advantageous.
  • A good understanding and application of the nursing scope of practice.
  • Sound knowledge of the scientific nursing process, nursing record-keeping, nursing procedures, pharmacology, patient health education and counselling, and Infection prevention and control (IPC) principles and protocols.
  • Develop/modify a nursing care plan skills.
  • Risk Management skills.
  • Patient assessment skills.
  • Proficient in Microsoft Office (Word, Excel and Outlook) and the ability to work on mobile applications.
  • Excellent communications skills (verbal and written).
Key Outputs
  • Your responsibilities include patient onboarding, crafting personalised care plans, and collaborating with allied healthcare professionals, delivering quality nursing care in collaboration with a multi-professional team.
  • As part of a nursing team, you will play a crucial role in leveraging wearable medical devices for real-time patient monitoring, ensuring seamless care through a 24-hour Care Collaboration Centre. Utilising cutting-edge technology, you'll support patients choosing home-based care over traditional hospital settings.
  • Identify, prevent and manage risks to ensure patient safety.
  • Facilitate a positive patient experience by creating a conducive care environment.
  • Provide accurate and comprehensive records of all care interventions.
  • Ensure personal growth through continuous professional development.
  • Ensure that equipment is maintained and all utilised items are accurately charted.
