Job Description
Job Summary Assist patients and clinicians, the Care CoOrdinator will patients and facilitate ongoing care of these patients in an appropriate setting, working alongside a multidisciplinary team including patient, family, and local community services. This requires the building and maintaining of excellent relationships with all levels of healthcare providers and patients. The Care Coordinator is responsible for scheduling, organizing, and managing all aspects of a patients healthcare maintenance and treatment. Duties include amongst others; helping patients complete paperwork, communicating with the healthcare team about a patients treatment plan and educating patients about resources and options for managing their health. Qualifications and experience required Clinical Associate or Registered Nurse: Bachelor of Clinical Medical Practice/ Degree in Nursing 5 years industry experience with a minimum of 2 or more of the following: o 3 or more years of clinical experience o Care Management or similar experience o Multidisciplinary team experience Valid HPCSA registration.
