POSITION AVAILABLE: CARE PLAN CO-ORDINATOR xe2x80x93 PERMANENT xe2x80x93 STELLENBOSCH The organisation is a facility that cares for individuals with Intellectual Disability. We are looking for a strong candidate that is able to develop and manage a holistic therapeutic solution for our residents in terms of including but not limited to, Individual Development Plans, Therapeutic Interventions, Life Skills Training and overall departmental coordination with competent reporting of all these elements. KEY RESPONISIBILITES

• Management of multidisciplinary team
• Monitor and evaluate team interventions.
• Professional development of MDT
• Individual Development Plans (IDPsxe2x80x99)
• Life Skills Training Plan
• Reporting on various levels
• External service provider reports
• Key role player in the training and development of staff
• Therapeutic intervention
• Eco Therapy
• Student placements
• Staff liaison
• External liaison/networking

SKILLS REQUIRED:

• Must have a passion for caring and working with individuals with intellectual disabilities
• Strong leader and management qualities
• Excellent computer skills
• Good interpersonal and communication skills at all levels xe2x80x93 families, residents and staff
• Highly motivated, energetic and able to handle pressure
• Able to coordinate inputs of multiple departments as well as external service providers

OTHER REQUIREMENTS

• Own Transport and valid Driverxe2x80x99s License
• Afrikaans and English (Read, write and Verbal xe2x80x93 Especially Afrikaans as most of our Residents are Afrikaans)

QUALIFICATION

• Qualification in Social Work, Psychology, or related field
• 10 Years experience in related field
