MHR is currently looking for an experienced

SOCIAL WORKER

for a

part-time shifts

in

Cape Town

in the Western Cape. Please note that our client just needs a social worker to step in should one of the permanent staff is sick ect. Therefore,

you can not be in a permanent position



Minimum Requirements



Bachelor of Social Work (BSW) degree Must be registered with the South African Council for Social Service Professions (SACSSP) 3-5 years of experience as a Social Worker, preferably in a healthcare or physical rehabilitation setting Knowledge of applicable Legislation Must have a valid Code 8 Driving License and own transport Strong interpersonal and communication skills Ethical decision-making Ability to work effectively in an interdisciplinary team

Duties and Responsibilities




Responsible for providing emotional support, counselling, and resource coordination for patients recovering from injuries, surgeries, or chronic conditions. Acting as a liaison between patients, families, and the interdisciplinary team, ensuring clear communication, facilitating access to services, and promoting a positive patient experience for optimal recovery and reintegration into daily life.


Key Performance Indicator:




Assess patient's psychosocial needs, provide emotional support, and advocate for patient rights. Act as a liaison between patients, families, and the interdisciplinary team to ensure clear understanding of diagnoses, treatment plans, and care processes Develop individualized discharge plans and coordinate home care, outpatient rehabilitation, and community resources Provide patients and families with coping strategies for illness, disability, and lifestyle adjustments. Educate patients on healthcare options, coping mechanisms, and available support systems Work with physicians, therapists, nurses, and Case Managers to create and implement holistic care plans Address urgent psychosocial issues, including financial hardship, housing insecurity, and mental health crise * Maintain accurate patient records, ensure compliance with healthcare regulations, and uphold ethical standards in practice.
