Key Performance Areas Patient Care Bereavement Care Spiritual Care Day Care 1. Develop and implement strategies to provide psychosocial support to patients and families, aligning with the mission and values of Tygerberg Hospice. 2. Collaborate with the Palliative Care Manager and interdisciplinary team to assess patients' psychosocial needs and create personalized care plans. 3. Lead the recruitment, training, and supervision of volunteers, ensuring a strong support network for patients and their families. 4. Coordinate and oversee the day care program, including planning activities, scheduling, and ensuring a comfortable and nurturing environment for patients. 5. Provide direct psychosocial support to patients who require specialized assistance, utilizing counselling techniques and therapeutic interventions. 6. Collaborate with external organizations, community resources, and mental health professionals to offer comprehensive psychosocial care. 7. Monitor and evaluate the effectiveness of psychosocial programs, making adjustments as necessary to meet patient and family needs. 8. Maintain accurate and detailed records of patient interactions and progress, adhering to confidentiality and ethical standards. 9. Assist in educational initiatives for staff, volunteers, and the community to raise awareness about psychosocial care and palliative support. 10. Contribute to fundraising and grant-writing efforts to secure resources for the expansion and improvement of psychosocial services.
