Job Description


Main purpose of the job:

  • Responsible for the oversight of the Gophelega DMOC as well as implementation and performance monitoring
Location:
  • Pretoria xe2x80x93 Tshwane
Key performance areas:
  • Oversee and provide technical advice on the implementation of DMOC in Tshwane district
  • Support facilities, community and technical team staff with the implementation of the differentiated model of care (DMOC) in the allocated clusters and facilities
  • Support in developing work plans/strategic plans for respective programmes and working closely with each unit for the implementation thereof
  • Fully accountable for the programmexe2x80x99s performance and provide strategic guidance and technical assistance to the DMOC staff and contract CBOs
  • Work closely with the retention, PSS/Retention, HAST team, PHC and District Pharmacist to fully provide guidance to the DABLAP Meds/ CCMDD in line with the National policy
  • Compile Monthly, Quarterly, and Annual reports, and presentations according to programme Objectives, specifications and requests for submission to senior managers
  • Participate and coordinate work plans for the DMOC section and facilitate integration into the overall program including participation in the future direction of the project together with the seniors of the Programme
  • Contribute towards development, monitoring and evaluation methods and tools for the programmes
  • Communication and integration of the project strategy with all stakeholders and programmes
  • Coordinate inputs from the district health management regarding the project strategies
  • Support implementation of decanting adherence quality improvement programmes within the clinic and achieve optimal clinical outcomes for patients with chronic communicable and non-communicable diseases (NCDs)
  • In collaboration with the District, identify and support external pick-up points
  • Facilitate implementation of national adherence policy and assess and support the district (facilities and community) on policy implementation
  • Support and monitor the functionality of the adherence clubs in line with COVID-19 regulations
  • Work with M&E teams and health facilities to effectively identify patients eligible for decanting and ensure that they are enrolled and monitored in the DMOC
  • Conduct Site visits for support, supervision and implementation of the DMOC intervention
  • Assist with the identification, writing, monitoring, implementation and expansion of Quality improvement projects
  • Conduct and attend regular and ad hoc meetings with clinics and other stakeholders to discuss and monitor the program including escalation of issues to the next level if not resolved
  • Identify training gaps at facilities and arrange with the training department the necessary training
  • Take ownership and accountability for tasks and demonstrate effective self-management
  • Follow through to ensure that quality and productivity standards of own work are consistently and accurately maintained
  • Take ownership of driving your own career development by attending training and development sessions and relevant forums/meetings
  • Maintain and update knowledge of developments in the field and expertise
Required minimum education and training:
  • Degree/Diploma in Nursing or Health Sciences qualification or equivalent
Required minimum work experience:
  • Minimum 5 years experience in project/programme management, preferably within the public health sector
Desirable additional education, work experience and personal abilities:
  • Experience in public health will be an advantage
  • HIV Management qualification, Knowledge of the new/updated health policies and guidelines, Health Information Management knowledge (Data analytic skills and knowledge) and M&E skills
  • Good communication skills
  • Experience in implementing independent projects
  • Familiarity with the district health system
  • Good administrative skills with working knowledge of Microsoft Office
  • Able to work under pressure and adhere to deadlines
  • Self-motivated, able to work independently and work as part of a multidisciplinary team
  • Assertive, confident, and adaptable
Demands of the job:
  • Travelling between facilities in the Regions of the allocated district
  • Working in rural areas or under-resourced circumstances may be experienced from time to time
  • Work in a highly pressurised environment
Communications and relationships:
  • Maintaining effective working relationships with the district, sub-district and facility staff is expected
  • Liaise effectively with relevant stakeholders throughout the district, NGOs, and the Department of Health
