Job Description

FAMSA Western Cape seeks to employ a Professional Nurses on a one-year contract position based at Observatory Cape Town.

The Western Cape Health Care 2030 vision is a strategic plan for the Western Cape government to improve the healthcare system and provide quality, accessible, and affordable healthcare services to all its citizens by 2030. This patient-centred approach uses the Community Oriented Primary Care (COPC) model, that emphasizes the role of the primary care provider in improving the health of communities. FAMSA WC's Integrated Community Wellness Project offers patients primary care services as the first link in the Health Care System, operating in the designated Nodes of Greenpoint and Woodstock.

Job Requirements:



Professional Nursing Qualification (certified copy needs to be provided) Up-to-date registration with the SANC (certified copy needs to be provided) Driver's License. (certified copy needs to be provided) Police clearance Use of own personal vehicle. 5 years Professional Nursing Experience (Experience in community based care will be an advantage) English and Afrikaans and/or Xhosa speaking Planning, coordinating, organizing, and negotiating skills. Effective communication and interpersonal skills. Able to work well in a team and independently. Self-management. Able to take initiative. Administrative and Computer Skills. Attention to detail and comprehensive in completion work tasks. Meeting of deadlines. Adaptability and flexibility. Responsibility and reliability with use of official work time. Emotional maturity / stability.

Key Responsibilities:



Reporting to the FAMSA WC CBS Manager. Professional Nurse duties as required by

Community Oriented Primary Care (COPC) model

including, but not limited to, administering injections, taking blood, caring for wounds and performing personal patient interventions, interpreting of patient information and issuing medication and other duties required by the DOH CBS programme. In addition, performing administrative duties working in collaboration with other health professionals. Visiting newly referred patients (including self-referred) at home within 48 hours. Documenting findings and drawing up a Care Plan. Allocation to the carer and ensuring that the patient is entered into HBC register. Checking the HBC register and making sure outstanding information or category change is entered. Visit all Category 3 patients at home for reassessment and make changes to the care plan if necessary, documenting the follow-up date. Ensure that all are visited. Complete the report on all activities and submit it to the designated DOH Manager. Complete the monthly stats form and submit to the designated DOH manager by the 5th of each month. Order stock. Complete and submit stats. Supervision of carers. Complete and document supervisory visits. Ensure there is an updated Community profile with identified community resources - updated quarterly. Visit patients alone (without CHW) unannounced and monitor their care experience. Refer patients back to PHC as necessary and appropriate. Visit patients at home who are very ill or whose condition is deteriorating. Provide support to CHW as necessary. Document each visit to a patient's home and document. Attend all meetings arranged by Sub-district CBS Co-Ordinator. Collect time sheets from carers and total up. Actively promote and participate in bringing about the Department of Health's vision of COPC . Provide support and guidance at the Wellness Hub Ensure the activities are documented, and that the services remain relevant. Meeting with carers as necessary to provide: in-service training, clinical updates, and programmatic information. Conducting family planning in line with the latest training, norms and standards. Participate in the Facilities Health Committee - Ensure the committee is kept abreast of the community's health needs and practical ways they can support CHWS. Follow the detailed guidance on the Essential activities list by NPO OTL in achieving the above.
