Professional Nurse
Mid Career (Above 6 yrs exp) We are seeking a Professional Nurse to provide comprehensive nursing care to patients with life-limiting illnesses. As a key member of the team, you will collaborate with interdisciplinary teams, provide emotional support to patients and families, assess referrals, manage admissions, and develop staff off-duty rosters.
- Deliver comprehensive nursing care to patients with life-limiting illnesses
- Collaborate with an interdisciplinary team to develop and implement plans
- Provide emotional support to patients and their families
- Assess and coordinate new referrals
- Manage admissions and discharges
- Plan and manage staff off-duty rosters
- File audits
- Attend Monthly meetings
- Cross functioning between the Inpatient Care Facility and Home Nursing program
- Willingness to work overtime, day and night duty, weekends and ph to meet operational requirements
