THE JOB AT A GLANCE
As the Onsite Case Manager, you will oversee the effective management and transition of injured workers from acute hospital settings into Rand Mutual Assurance's dedicated sub-acute rehabilitation facilities. Your role directly supports RMA's strategic goal of providing specialised occupational rehabilitation, enhancing patient outcomes, and increasing the utilisation of RMA's rehabilitation infrastructure under the Compensation for Occupational Injuries and Diseases (COID) framework. Additionally, as the Case Manager you will act as a primary liaison between RMA, employers, injured workers, and their families, ensuring clear and consistent communication throughout the rehabilitation process. Your role will be primarily based onsite at acute hospitals, with regular visits to RMA Head Office and RMA sub-acute rehabilitation facilities.
WHAT WILL YOU DO?
Patient Management and Coordination

  • Identify injured workers in acute hospitals suitable to transition to RMA's sub-acute rehabilitation facilities
  • Conduct regular hospital rounds to monitor patient progress and readiness for rehabilitation
  • Coordinate timely and seamless patient transfers to RMA's sub-acute facilities.
Clinical Assessment and Authorisation
  • Conduct thorough clinical assessments in consultation with the acute hospital multidisciplinary teams to determine patient eligibility for rehabilitation services
  • Liaise with Office Case Managers to coordinate authorisation for patient transfers in alignment with RMA's clinical protocols and COID guidelines
  • Ensure efficient transitions from acute care to rehabilitation, minimising unnecessary acute hospital stays.
Stakeholder Engagement
  • Develop and maintain professional relationships with treating physicians, hospital staff, allied health professionals, occupational health teams, injured workers, and their families
  • Provide clear guidance on eligibility criteria, admission procedures and the rehabilitation services provided by RMA
  • Actively promote RMA's sub-acute rehabilitation facilities among acute hospital clinical teams, highlighting clinical outcomes, cost-effectiveness, and patient recovery advantages
  • Foster and maintain relationships with acute hospital teams, occupational health specialists, and other key stakeholders to enhance patient referrals and utilisation rates.
Liaison and Communication
  • Serve as the central communication link between RMA, injured workers and their families
  • Clearly communicate patient progress, rehabilitation plans, expected outcomes, and anticipated timelines
  • Address concerns, answer queries, and ensure all parties remain informed throughout the rehabilitation journey.
Monitoring and Reporting
  • Accurately document clinical progress for patients in acute, patient transfers, and rehabilitation progress

WHAT YOU'LL BRING TO THE TABLE?
Skills and competencies
  • Registered Professional Nurse with 3-5 years' experience in Trauma or ICU
  • Managed Care experience is an added advantage
  • Desirable: A good understanding of Compensation for Occupational Injuries and Diseases Act (COID Act) healthcare funding processes and rehabilitation pathways
  • Excellent interpersonal, communication, and stakeholder management skills
  • Strong clinical judgement and decision-making capability
  • Proactive approach to patient coordination and case management
  • Ability to manage diverse stakeholder expectations effectively.
