Job Summary Key Performance Areas/Key Responsible Areas a) Manage all primary healthcare patients referred to by The Medical Society clinicians. b) Initiate and analyse treatment plans. c) Explain and discuss test results or prescribed treatments with patients. d) Monitor patient progress and response to treatments. e) Diagnose and manage chronic disease patients (in accordance with the designated scheme rules and protocols). f) Refer patients to other providers (where appropriate) g) Tele-consult with patients when outside of the nurses scope
