Job Description

This position supports a USAID-funded program operating across three districts-Ehlanzeni, Nkangala, and Gert Sibande-in Mpumalanga Province. Led by Pact and supported by mothers2mothers (m2m), the initiative focuses on delivering vital community-based services to children living with HIV (CLHIV), 0 to 20 years old, as well as their families. Working closely with government bodies like the Department of Health and other key partners, this role plays a crucial part in reaching and retaining these vulnerable populations throughout the HIV care continuum. The project's implementation is planned from July 1 to September 15, 2025, with dates potentially subject to adjustment.
Key Performance Areas:
Partner/Stakeholder management:

  • management in the health facility and community
  • Participate/represent M2M in the government department's (DOE, DOH, DSD, LACs, CSOs etc.), stakeholders and other relevant partners' meetings at sub-district and district meetings, monthly and quarterly or as needed.
Coordination of linkages:
  • Provide assisted linkage to PrEP for all index contacts in discordant relationships and all other high-risk clients including AGYW/B with a negative HIV status, ART initiation, ART re-initiation, GBV survivors, high impact services, health services, child protection services etc.
  • Provide bi-directional reports to relevant stakeholders of all clients that you have supported for with linkage to service across the cascade.
  • Conduct quarterly Mentorship support to Family mentors and Case Coordinators
  • Identify, support enrolment and serving of XXX children and adolescents living with HIV in the ACHIEVER program by end of Q3 in CoP 23
  • Establish, conduct or support establishment and facilitation of Kids adherence support groups. For C/ALHIVs 10-14 + newly diagnosed/unsuppressed viral loads (Community/Facility).
Administration/ Recording and Reporting:
  • Weekly documentation of all linkages in the linkage register as per the weekly referral summary
  • Compilation of monthly reports that feed into quarterly and annual organisational reports.
  • Provide feedback reports on linkage activities
  • Participate in internal staff and other units' meetings.
Job Requirements
Qualifications and Experience:
  • Qualification in Nursing Assistant or equivalent experience (3-4 years) in Linkage Officer (or similar) role
  • Experience with OVC and/or adolescent health programming preferred
  • Knowledge of community -based health service delivery and case management models
  • Fluent in written and spoken English and competent at written communication and reporting writing
  • Proficient in basic computer skills
  • Knowledge of DOH systems and protocols
