Job Description


Main purpose of the job:

  • To promote and provide HIV Testing and Services, support Viral load and retention in care in line with the 95-95-95 strategy with the cluster/community and Differentiated Care Teams (DCTs)
Location:
  • Pretoria - Tshwane
Key performance areas:
  • Participate in demand-creation activities by educating and recruiting clients for HIV testing
  • Provide HIV information to clients that include but is not limited to risk reduction, ARVs, adherence, nutrition, hygiene, PMTCT, infant feeding, PrEP, and retention in care
  • Provide HTS sessions per different testing modalities e.g., HIVSS, Index testing, etc.
  • Ensure provision of HTS by team members in line with national HTS quality standards (participation in IQC, PT, and ensuring availability of quality resources in the facility)
  • Conduct HIV and IPV Risk assessments and refer appropriately e.g., PrEP, PEP, Psychosocial services, etc.
  • Link newly diagnosed patients to care and treatment and ensure that they are initiated on ART
  • Follow up with patients who were not initiated on ART to ensure that they ultimately initiated treatment
  • Coordinate with the WBPHCOT and CBOs linked to the facility to integrate HTS in their activities
  • Implement and monitor utilization of tools and registers e.g. Risk tools, Adherence forms, elicitation forms, etc.
  • Identify HTS disabling factors, resolve within scope, or refer accordingly
  • Keep track of PCR collection, results communication documentation, and capturing in Tier. Net
  • Follow up PCR-positive infants to ensure ART is initiated
  • Source from Data Capturers lists of patients who missed appointments
  • Audit files and communicate with NIMART patients eligible for medicine drop-off
  • Facilitate appointment/reminder calls by sharing lists with Treatment Navigators to call patients
  • Coordinate with the WBPHCOT and CBOs for integration of physical tracing of patients in their activities
  • Conduct physical tracing and medicine drop-offs to eligible patients
  • Confirm and update demographic information for patients successfully traced and re-enrolled into care
  • Provide TB screening and assess TPT eligibility at medicine drop-off
  • Provide enhanced adherence counseling and complete adherence plan for patients returning to care
  • Coordinate referral of complex cases to PSS mentors/senior clinicians
  • Follow up with post-natal women after 1 week to ensure they have been seen at the referral PHC facility and referred for tracing (through facility linkage officer or WHCOBOT Team Leader)
  • Screen for TB, STIs, contraceptive needs, and GBV/IPV using approved tools and guidelines and refer as necessary
  • Note and share with data capture changes of patient's demographic data for update
  • Work with the facilities and designated team managers and assist in the home ART delivery of medication
  • Obtain viral load due lists from data capturers and work with Treatment Navigators to send reminders to patients
  • Collect blood samples, and send specimens out to the laboratory using correctly filled NHLS forms with relevant EGK codes
  • Participate in VL Blitz activities in/outside facilities
  • Use E labs system to follow up results, update patients' records, and send for capturing on Tier.NET
  • Identify, track, and trace patients with abnormal viral load results and refer them for clinical intervention
  • Minimise waiting time/fast track patients called to the facility for VL collection
  • Ensure that HTS data (including other supportive data like Viral load, home ART delivery, etc.) is reported
  • Ensure and monitor that the data reported back to the facilities is reported and updated in the Tier.Net e.g., Viral load
  • Participate in weekly and monthly facility program performance meetings
  • Submit reports on required intervals (Daily, weekly, monthly)
  • Take ownership and accountability for tasks and demonstrate effective self-management
  • Follow through to ensure that quality and productivity standards of own work are consistently and accurately maintained
  • Maintain a positive attitude and respond openly to feedback
  • Take ownership of driving your own career development by participating in ongoing training and development activities such as conferences, workshops, etc.
Required minimum education and training:
  • Certificate in Enrolled Nursing with Phlebotomy experience
Required minimum work experience:
  • At least 1-year relevant experience in the Health Field, Phlebotomy, or related position in HIV/TB services
  • Minimum 2 years experience in Public Health
Desirable additional education, work experience, and personal abilities:
  • Excellent verbal and written communication and interpersonal skills
  • Teamwork and collaboration
Demands of the job:
  • Working overtime or over weekends may be required from time to time
  • Able to work in a highly pressurized and challenging environment
  • May be required to work at sites that are under strict confidentiality
  • Tact and discretion must be maintained at all times
  • Local travel may be required
