POSITION: Community Social Worker

LOCATION: MES Kempton Park Branch

REPORTS TO: MES Branch Manager

OVERALL PURPOSE OF THE JOB

The Community Social Worker will provide professional social work services to individuals experiencing homelessness and other vulnerable clients. This includes conducting intakes and risk assessments, developing individualized development and exit plans, offering psychosocial support, and facilitating access to relevant services. The role focuses on promoting client empowerment, family reunification, and sustainable reintegration into the community.

DUTIES & RESPONSIBILITIES

MAIN DUTIES AND RESPONSINBILITIES

Conduct client intakes, psychosocial assessments, and risk evaluations.

Develop Individual Development Plans (IDPs) and sustainable exit plans for all clients.

Maintain accurate and confidential client case files in accordance with MES and SACSSP standards.

Provide individual and group counselling to address clients' emotional, social, and practical challenges.

Facilitate life skills training and workshops that promote clients' social and personal development.

Collaborate with internal and external stakeholders to coordinate referrals and access to services.

Plan and implement interventions aimed at improving the overall well-being and resilience of clients.

Facilitate and manage family reunification processes through counselling, mediation, and family assessments.

Participate in team meetings, case conferences, and programme development initiatives.

Monitor, evaluate, and document client progress and outcomes through regular reporting.

REQUIREMENTS

Education/Qualifications

? Bachelor of Social Work Degree

? Proof of Registration with the SACSSP

Knowledge/Skills/Experience Desired

? Minimum of 2 years' post-qualification social work experience (experience in homelessness, community work, or related fields will be an advantage)

? Strong assessment, counselling, and case management skills

? Excellent report writing and communication skills

? Computer literacy (MS Office)

? Valid driver's licence and ability to drive

? Ability to work under pressure and meet deadlines

? Strong interpersonal skills and the ability to work effectively within a multidisciplinary team

Job Type: Temporary
Contract length: 6 months

Pay: R14000,00 - R15000,00 per month
