Case Manager (Remote)

In This Role, You'll:

Review referrals and gather supporting documentation, including medical records and labs. Collaborate with healthcare providers, home health agencies, and insurance companies to coordinate comprehensive, patient-centered care plans. Communicate directly with patients: conduct clinical interviews, collect health histories, and guide completion of paperwork. Track, document, and manage patient cases through our EMR and digital tools--ensuring up-to-date records and regulatory compliance. Provide ongoing support throughout the patient journey, acting as a liaison and advocate for needed services and resources. Monitor care plans, assess outcomes, and proactively manage follow-ups to optimize healing and satisfaction.

You're a Great Fit If You:

Have a Bachelor's degree in Nursing, Social Work, Healthcare Management, or related field (Master's preferred). Have 2-4 years in care coordination or case management, ideally with wound or chronic care populations. Have an RN license which is required for nursing professionals. CCM or eligibility a plus.

Bonus Points If You:

Are adept in EMR/case management software, insurance and healthcare systems, and multidisciplinary collaboration. Have exceptional communication and organizational skills. Are driven by integrity, patient-first advocacy, and have a solutions-oriented mindset.

Pay: $1,300 per month

Work schedule: US Timezone (East Coast)
