The Role: Care Plan Facilitator (Senior Practitioner)



You will be the central guide for our clients. Your mission is to conduct a holistic needs assessment, design a personalised support plan, and coordinate a team of care professionals to bring that plan to life.

This is an end-to-end role:

Holistic Assessment:

You will meet with clients and their families to understand their medical, social, emotional, and practical needs.

Plan Design:

You will architect a comprehensive support plan that outlines the goals, interventions, and required services.

Stakeholder Coordination:

You will be the link between the client, their family, support workers, therapists, and doctors, ensuring everyone is aligned.

Advocacy & Navigation:

You will help clients understand and access their rights and the services available to them within the South African landscape.

Monitoring & Review:

You will own the client's journey, ensuring the plan is effective and making adjustments as needed.

Who We Need:



You must be:

A qualified professional (Degree in Psychology, Social Work, or a related field). SACSSP registered (or eligible for registration). An expert in client-facing assessment and planning. A phenomenal communicator and natural problem-solver. Exceptionally organised and proficient with digital tools. Resilient, autonomous, and driven by a need to see real-world impact.

Gate 1: The Builder

(Company Revenue < R25M)

Hourly Rate:

R80/hr

Plus:

7.5% commission on the first week's support plan for every client you onboard.

Your Mission:

Build the system from zero.

Gate 2: The Leader

(Company Revenue >= R40M)

Hourly Rate:

R160/hr

Plus:

7.5% commission on your clients + a 2.5% override on your team's clients.

Your Mission:

Lead and scale a team.

Gate 3: The Partner

(Company Revenue >= R62.5M)

Hourly Rate:

R320/hr

Plus:

Equity stake.

Your Mission:

Lead the department and partner with us to launch your own practice.
